The infertility study is performed in couples who after a year having unprotected sex, have not achieved pregnancy. This type of tests is the first step to determine if there is any cause of infertility. At Clínica Medrano we know that each patient is unique, so fertility tests are carried out in a personalized way, taking into account the characteristics of each couple.
The most frequent diagnostic tests are:
- Seminogram or spermiogram: Semen analysis to know the characteristics of sperm (quantity, mobility and shape), aspects that influence achieving pregnancy. It is the first phase of the study of fertility.
- Hormonal analysis of the woman: It consists of a blood test that is performed in the first days of the cycle in order to determine the functional status of the ovary and pituitary gland.
- Hysterosalpingography: This is an x-ray of the pelvic area that is performed in contrast with the objective of determining if the fallopian tubes are obstructed.
ARTIFICIAL INSEMINATION (AI)
Artificial insemination is a simple and minimally invasive assisted reproduction technique that consists of the introduction of a sample of semen in the uterus of the woman with the aim of favoring fertilization, facilitating the encounter between the sperm and the ovum. Unlike in vitro fertilization, egg extraction is not performed.
The treatment begins with the ovarian stimulation of the woman with the aim of increasing the chances of pregnancy, and a necessary endometrial control is performed to determine the perfect moment for the Artificial Insemination. Subsequently, the semen sample is prepared by eliminating those slow, immobile, or dead sperm, optimizing the quality of the semen that will be used for the treatment. Finally, we proceed to insemination. After this procedure, the patient will receive a hormonal medication in order to favor implantation and multiply the chances of success.
There are two types of artificial insemination depending on the origin of the semen: the Conjugal Artificial Insemination (CAI), in which the sperm deposited in the patient comes from its partner, and the Artificial Insemination by Donor (AID), in which a seminal sample from an authorized sperm bank is used.
IN VITRO FERTILIZATION (IVF)
In vitro fertilization is an assisted reproduction technique that consists in the fertilization of an egg with a sperm in a laboratory to obtain embryos that are later transferred to the woman’s uterus. The first step in an IVF treatment is ovarian stimulation, through which we will get more oocytes that will lead to more embryos, multiplying the chances of achieving pregnancy. Later, when it is considered that the adequate number of ovules is available, it is extracted by means of an intervention called puncture. When we have the ovules and the sperm, the next step is the fertilization of the ovules, placing this in a culture plate surrounded by sperm to fuse. The resulting embryos are studied and the best ones are selected to transfer them to the uterus
With the embryos that have not been introduced in the maternal womb, their vitrification is carried out with the aim that they can be used again, either for a new attempt, or for a second pregnancy.
MICROINJECTION OF SPERM
The technique of sperm microinjection or IVF-ICSI is an in vitro fertilization technique in which a live sperm is introduced into an ovule by puncturing it, with the help of a pipette. Unlike conventional in vitro fertilization, with this method the sperm hardly makes any effort, so it can be carried out with samples of very low quality.
IVF-ICSI is recommended in cases in which semen quality is low, when sperm are obtained directly from the testicle (either by obstruction of the ducts, ejaculation problems …), risk of infectious diseases of the male or for azoospermic patients.
The procedure performed to proceed to in vitro fertilization by microinjection of sperm is identical to conventional IVF so the steps to follow are: ovarian stimulation, extraction of ovules by puncture, fertilization of the ovules, study of embryos to select the best ones, and their introduction in the maternal womb.
PRESERVATION OF FERTILITY

From the age of 40, the biological clock slows down and the chances of pregnancy are greatly reduced. Having the option to postpone motherhood without fearing the passage of time is a great advantage for a large group of women who for personal or health reasons can not or do not want to have children today.
This type of treatment is indicated especially for patients diagnosed with cancer who are going to receive chemotherapy or radiotherapy treatments or women who are at risk of loss of ovarian function. In addition, it is also applicable to women who for personal or work reasons decide to postpone their motherhood until they consider that the perfect time has come to be a mom.
PREIMPLANTATION GENETIC DIAGNOSIS (PGD)
Preimplantation genetic diagnosis (PGD) is a complementary technique to in vitro fertilization by sperm microinjection (IVF-ICSI), which consists in the analysis of the embryos after they have been fertilized and before being transferred to the uterus, in order to prevent the birth of babies with genetic or chromosomal diseases.
On the third day of fertilization, the ovum using the sperm microinjection technique (ICSI), an embryo biopsy is performed to rule out those pre-embryos that are at risk of suffering from any pathology. This procedure is performed without altering the development of the embryo. Subsequently, the medical team decides which pre-embryos will be transferred to the maternal womb.
PGD is applied mainly when some member of the couple is a carrier of some disease or genetic anomaly or have the number of chromosomes altered, when there is a family history of disease or genetic alteration, when the maternal age is advanced or when the patient has suffered several abortions
Thanks to the generosity of our patients, at Clínica Medrano we have an authorized embryo bank from couples undergoing IVF-ICSI treatments. Those embryos that have not been transferred to the woman are conserved to be donated to other couples for reproductive purposes or for research, always with the consent of the patient. These embryos come from completely healthy patients, and from women under 35, as stipulated by law.
